Home-Based Care Nurse

We are seeking a qualified Freelance Home-Based Care Nurse to provide nursing support to clients recovering at home, elderly patients, and individuals living with chronic illnesses or special care needs. This position is ideal for a nurse who prefers flexible assignments and is experienced in delivering care in non-hospital settings.

The role involves visiting clients at their homes to monitor health conditions, assist with wound care, administer medications as prescribed, support post-operative recovery, provide palliative care support, and educate family members on proper patient care. You will also be responsible for observing changes in the patient’s condition and reporting any concerns promptly.

The successful candidate must demonstrate professionalism, confidentiality, empathy, and excellent clinical judgment. Assignments may vary depending on client needs and can include short-term or ongoing care arrangements. This role requires a nurse who is reliable, mobile, and able to maintain high standards of care while working with limited supervision.

Diploma or Degree in Nursing from a recognized institution
Valid license from the Nursing Council of Kenya
Minimum of 2 years’ experience in clinical nursing, home-based care, geriatrics, post-operative care, or palliative care
Ability to assess patient needs and respond appropriately in home settings
Knowledge of medication administration, wound care, infection prevention, and basic emergency response
Strong interpersonal and communication skills
Ability to maintain patient confidentiality and professionalism at all times
Reliable, punctual, and able to work with minimal supervision
Must be willing to travel to clients’ homes within the assigned area
Experience supporting patients with chronic illnesses such as diabetes, hypertension, HIV, or cancer is an added advantage
